Investigators determined that the elderly man was driving through the north traffic lane, near Rincon Reservation, in a Ford Ranger truck, when he approached a road bend.
For unknown reasons, the pickup swerved into oncoming lanes, rather than continue past the curved path. The wrong-lane vehicle grazed the side of a southbound Chevrolet Tahoe, galloped past it, and dashed into an oncoming Chevrolet Silverado.
First responders attempted to rescue the operator of the Ford, a San Diego resident, from the Mira Mesa neighbourhood, but he never made it to the hospital.
The occupants of the two other cars involved survived the crash. While the female driver and sole occupant of the Tahoe was relatively unscathed, the two commuters in the Silverado, both Vista residents, were hospitalised on complaints of pain.
Detectives of the North County Police Department are yet to determine the cause of the deadly incident as investigations are still being conducted.
