The authorities said that the events that led to the accident started after a 2002 Ford flatbed construction truck traveling south on Pacific slowed down to make a turn on Deer Creek. At the same time, Carrillo, who was driving a 2005 International box truck was coming behind the first vehicle.

The CHP said that the deceased failed to reduce his speed and rear-ended the Ford. The impact of the crash caused the vehicle to go into the northbound lane, where it collided with a 2011 Subaru SUV. Carrillo sustained severe wounds that claimed his life at the scene, and the medics pronounced him.

The driver of the Subaru sustained major wounds, while that of the Ford had moderate injuries. The medics took the two unidentified motorists to Ventura County Medical Center for treatment. The three vehicles involved were severely wrecked, and the authorities towed them away from the scene.

The accident report showed the CHP have not cited anyone for the accident, and the investigation is ongoing. Anyone with helpful information should contact the Ventura Area CHP on its listed line.
